Cap resistances, layer defenses and respect boss telegraphs.

Cap fire, cold and lightning resistance at 75% before maps. The game quietly lowers them by 10% at each stage of the campaign — by endgame you're 60% down — so you need about 135% on gear and passives to show 75%. Chaos resistance is not affected. Uncapped resistances are the most common reason for sudden deaths.

Layer defenses: armour or evasion, plus energy shield or life, plus a recovery source.

Bosses telegraph big attacks; dodge roll is your main defense

Upgrade your weapon regularly with crafting currency; damage from weapon outpaces gems early.
